Introduction

Welcome to Harrogate Private GP Ltd ("we," "us," or "our"). These Terms of Service ("Terms") govern your use of our general practitioner services ("Services"). By accessing or using our Services, you agree to comply with and be bound by these Terms.

Acceptance of Terms

Your use of our Services constitutes your acceptance of these Terms. If you do not agree to these Terms, please do not use our Services.

Eligibility

To utilise our Services, you must be at least 18 years old or have the consent of a legal guardian.

Description of Service

We provide general practitioner services at the convenience of your home within the Harrogate area. Our Services include consultations, assessments, investigations, prescriptions, and general medical advice. If the patient is under 18 they require the presence of their legal guardian at the time of the visit, although this may be in another room.

Appointments and Cancellations

Appointments must be booked in advance. This is generally done through the practice website. We reserve the right to charge a cancellation fee for any appointment cancelled less than 24 hours in advance before the scheduled visit. Appointment availability is liable to change at short notice due to nature of the clinicians availability but they will keep the website information up to date.

Fees and Payments

Fees for our Services are as stated on our website or provided upon inquiry. Payment is due at the time of booking the service unless otherwise agreed upon. Any additional payments due at the time of the visit should usually be paid via the point of sale device the clinician will have available. The clinician may be able to accept cash and will provide a receipt for this. If payment is not able to provided at the time of the consultation the patient/guardian has 14 days to provide this payment.

Medical Records and Confidentiality

We maintain records of the medical services provided to you. Such records are confidential and will be maintained in accordance with UK law, including the Data Protection Act 2018 and the UK General Data Protection Regulation (UK GDPR). We require access to your medical record - this may be via SystmOne, Emis, Airmid or your Summary Care record. We will ask you for consent to access this at each consultation and prior to your consultation. You can withdraw this consent at any time. Unfortunately, without access to your medical record we will be unable to offer you medical care.

Limitation of Liability

To the fullest extent permitted by law, we shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses.

Indemnification

You agree to defend, indemnify, and hold harmless Harrogate Private GP Home Visiting Service and its employees from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses.

Governing Law

These Terms shall be governed by and construed in accordance with the laws of the United Kingdom.

Changes to Terms

We reserve the right to update these Terms at any time. Any changes will be posted on this page.
